The Stress Buster Challenge survey is provided in two parts. The first prior to participation in the challenge and the second following completion. You do not have to participate in either part however, survey participants will be eligible for 500 Live Better rewards points for completing the pre challenge survey and 500 Live Better rewards points for completing the post challenge survey. Post challenge survey points (500 Live Better points) will only be awarded to those who have enrolled in and completed the Stress Buster Challenge, and completed the post challenge survey. The 500 Live Better rewards points will be awarded within 30 days of the completion of the challenge.
The survey is provided by Live Better Management (ACN 003 457 289) and Medibank Private Limited (ACN 080 890 259). If you participate in either or both parts of the survey Live Better and Medibank will collect your personal (including health) information and will use that information to understand changes in health knowledge and behaviours as a result of the challenge, and to improve this and similar Live Better challenges as well as future stress management support or programs.
Live Better will also collect your membership number and demographic information from Medibank for this purpose and so that pre challenge and post challenge survey results can be matched.
Your information will be de-identified to protect your privacy once it is no longer necessary for Medibank and Live Better to retain.
